Design-Your-Own Boxes are available in 5 sizes based on the number of items you choose. HERE'S HOW IT WORKS.

- Print out the cheese list and shipping information form.
- Fill out your selections, and shipping information forms.
- Fax both forms back to us: 262-785-6642. We will call you back to confirm your payment information.
- OR Call us toll-free: 877-820-3215. Please have your list of selections ready to go. Our phone order hours are: 9:00am to 7:00pm (CST) Monday though Saturday, and 9:00am to 6:00pm (CST) on Sunday.
Shipping Costs
1-2 items $16 for shipping and handling*
3-4 items $20 for shipping and handling*
5-6 items $25 for shipping and handling*
7-9 items $30 for shipping and handling*
10-12 items $35 for shipping and handling*
